The following is a list of the most common fees charged for services that are not insured by OHIP. The fees are charged in accordance with recommendations from the Ontario Medical Association and are subject to change without notice.
Complex Doctor's note

Driver's Physical Examination
Twinrix/Havrix or travel immunization
$20 per injection (does not include cost of vaccine)
Employment physical examination
HPV (Gardasil) vaccination for girls/women between the ages of 9 and 26 years
$190 per dose
$100 (depending on size of chart)
Fees must be paid in full prior to release of forms/notes.

General Clinic Policies:
The following are our office's general policies. Please check here often as they may be updated and may change without notice.
Please be courteous to our office staff. It is not easy to look after nearly 2,000 patients. We understand when you or a loved one is not feeling well times can be challenging, but we appreciate your patience with us as we try our best to accomodate your requests. We do reserve the right to discharge patients from our practice who consistently are rude, or mistreat or abuse our staff.
Appointment Times. We generally are able to accomodate routine appointments within 2 weeks. If you have a cold, are very sick, or need a prescription renewed, we can sometimes accommodate same-day appointment requests but we reserve the right to use our discretion.
Prescription Renewals. When you pick up your last refill from your pharmacy, be sure to make an appointment with us as soon as possible so you don't run out of your medications. We DO NOT ENCOURAGE renewal of medications over the phone but if it is simply not possible to do this, there is a $20 fee payable by credit card. There is no charge to you if you come to the office in person for prescription renewal.
Calling/Contacting our Office. We recommend that you use the "Online Inquiry" or "Request Appointment" links from our website for routine inquiries (e.g. lab result inquiries, test/referral bookings, etc.) and appointment requests. This will help keep our office operating smoothly. Please be sure to let us know your up-dated email address, and ask us to add you to our general email distribution list for important announcements.
Grounds for Discharge from Practice. The following are what we consider grounds for discharge from our practice:
Frequent rude or demanding behaviour to our staff.
Frequent missed appointments (more than 3) without providing 24 hours notice.
Frequent failure to attend follow up visits when requested.
Causing unreasonable difficulty to our staff insofar as it causes delay or disruption to the normal flow of our medical practice regarding the care of our other patients.
